Go to HKEY_CLASSES_ROOT/Applictions and look for "NoOpenWith" when you highlight any of the Open With List under Applications. If you have that entry then delete it. That entry will be in many of the Applications there and so will "No StartPage. Right now just look for those 6 entries you listed.

Go to the same place, look at Notepad.exe and see if if says NoOpenWith.

Even right clicking an AVI and "open with" does not call up the dialog.

Forget about right clicking for the moment.
What happens when you double click any file ??

It sounds to me like you have been enabling the "Always use the selected program to open this kind of file" option, and therefore most of your file types already have a file association, thats why you don't get the "open with" dialog box.

Well, i'm able to open a AVI file by double clicking - but used to have to option to be able to choose what I want to open it with by right clickiing. This uxl file is not registered and I see this by double clicking and no dialog at all with right click "open with"

*edit - "open with" did work for opening a .js file and some other files I don't normally use.

You need to open Windows explorer, and check file associations.
Tools > Folder options > file types tab.

Just remember not all "file associations" are registered. You will normally have to set them back to default or unknown to get the "open with" box back.
"open with" did work for opening a .js file and some other files I don't normally use.
The reason you get the "open with" option with those files is because you haven't set a file association for them.

I have set .js script to open with notepad but I still get other options if I right click.
With files like AVI I do not have the options. I have reset it to open with WMP but stilll should have the option to open it with other players as well.

I have always had the options before - no matter what file type it was - windows would say something like "unknown file type" and would ask me what to do - but does not anymore.

Are you running any tweak programs, like TweakXP,etc ??

You may have enabled something in one of those applications that is overriding the standard XP settings.

I have not run any tweak programs lately but noticed some changes after trying to play "funny" movies that would crash explorer.

It might be related to all the graphic modifications you use like themes, skins, etc. Try using XP's default skin/theme.
